The 11 Sections of the Adult Disability Report

One of the longest parts of your Social Security disability benefits application process will be filing the adult disability report. This 11-section report focuses on obtaining and organizing information regarding your work history and disability status. When filing for Social Security disability benefits you should be familiar with what the sections will ask.

Section 1 - Basic Information:
general information on you including name, address, and basic demographics.

Section 2 - Contacts:
basic information on another person who is familiar with your medical condition, disability and work history that may be contacted in the event further information is needed.

Section 3 - Medical Conditions:
a basic overview of your medical conditions and current health status.

Section 4 - Work Activity:
information on your current work status and why you are or are not working.

Section 5 - Education and Training:
your past educational history and any special education classes in which you may have been involved.

Section 6 - Job History:
employment history and job descriptions used to determine your work skills and abilities.

Section 7 - Medicines:
a basic listing of all the medications you are taking, the doctor who prescribed them, and the reason they were prescribed.

Section 8 - Medical Treatment:
the longest section of the report, this section deals with the treatment related to your disability. It is very important that you complete this section with accuracy to help your application go through the approval process faster.

Section 9 - Other Medical Information:
information on other organizations that may have details on your disabling condition such as Workers' Compensation or insurance companies.

Section 10 - Vocational Rehabilitation, Employment, or Other Support Services:
this section asks for any other support resources you may be currently using or receiving.

Section 11 - Remarks:
space to write additional information that may be beneficial to your claim, such as clarification or additional documentation you could not include in the above sections.
