A thorough review of the claimant's medical evidence of record relates that the claimant was involved in a car accident in March 2007, and suffered some bleeding in the brain which caused difficulty with memory. The documentary evidence of record also shows that the claimant experienced a stroke and as a result, she experiences seizures and headaches. However, following this the claimant was hospitalized at Columbia Hospital, covering the period May 24, 2007, through May 27, 2007, with a diagnosis of seizure disorder, hypertension, and status post right temporal cerebrovascular accident. At this time, the claimant underwent CT scan of the head which showed that there was suspect mass in the right temporal lobe along with a meningioma. A follow-up MRI of the claimant's head revealed volume loss and atrophy of the right temporal lobe, compatible with prior ischemic insult. The claimant also had an echocardiogram which showed an ejection fraction of 58%. It was also noted that the claimant was seen in consultation by the neurology services which indicated that the claimant was having seizures secondary to the temporal lobe infarct. The claimant was discharged with the following medications; Aspirin, folic acid tab, Tenormin, and Lamictal (unexhibited). Additionally, the claimant underwent routine treatments at Palm Beach County Health Department, since at least May 2007. Progress notes show that the claimant was treated for hypertension, seizure disorder, and history of cerebrovascular accident, throughout the duration of her visits, through October 8, 2007 (unexhibited). Nonetheless, on October 31, 2007, the claimant was treated at JFK Medical Center, for recurring seizures. At this point, the claimant underwent CT scan of the head which revealed patchy hypodensity within the right temporal lobe which is of unclear etiology and may reflect previous infarct. A 1.1 x 0.7 cm calcified extra-axial structure was seen in the right frontal region which is thought to represent a calcified menigioma (unexhibited). Furthermore, on January 9, 2008, consultative examiner, Dr. Mark Rogovin, D.O.'s report showed that the claimant has a history of hypertensive hemorrhagic CVA, with some short term memory loss and nonspecific weakness and complications including seizures and headaches with a likely preexisting history of hypertension and hyperlipidemia with poorly controlled blood pressure (unexhibited).
