She was also morbidly obese. The progress notes indicate that the claimant's blood pressure was elevated. The claimant was hospitalized in March 2008 for a chief complaint of right upper extremity numbness. CT scan of the shoulder demonstrated degenerative changes with spur formation in the AC joint. She also had mild cervical spondylosis at C5-6. After suffering severe trauma to her right upper and lower eyelids, the claimant underwent reconstruction of the right lower lid with repair of laceration in June 2008. When followed up by her ophthalmologist, the claimant reported decreased vision and diplopia, however, there was no evidence of diabetic retinopathy or other ocular trauma. Her diplopia was likely due to her orbital fracture and associated manipulation. The medical evidence establishes that the claimant has been treated for a number of medical conditions that severely impact her functioning, including diabetes with neuropathy, hypertension, and shoulder pain. In addition, she suffered a right eye trauma that bas resulted in diplopia and blurred vision. The claimant's morbid obesity, given her recorded weight of 365 pounds in May 2008, severely impacts her mobility and her chronic conditions, including her diabetes and hypertension. The claimant has been prescribed multiple medications, including Singulair, Prilosec, Neurontin, Elavil, HCTZ, Albuterol, ibuprofen, Accupril, Norvasc, and insulin to control her conditions. The Administrative Law Judge finds that her numerous medical conditions significantly impacts her ability to function and prevents her from performing sustained work activity.
After considering the evidence of record, the undersigned finds that the claimant's medically determinable impairments could reasonably be expected to produce the alleged symptoms and that the claimant's statements concerning the intensity, persistence and limiting effects of these symptoms are generally credible. The claimant testified that she has lower back pain that shoots down her right leg to her knee that makes it difficult for her to walk. She has circulation problems with numbness and must shake her hand and leg to get her circulation going. She is diabetic and uses insulin in the morning and evening. Her blood sugar was 450 in the morning. She has trouble seeing, sees double, and has floaters because of her diabetes. She was hospitalized for one week for diabetes this past year. She also had a heart attack in April 2008. She uses a nebulizer at least twice a day. The claimant also stated that she hallucinates and hears voices when she is alone.
